Anyone with proper identification, regardless of age, may apply for a Fort Berthold Reservation Library borrower's card. Once a card has been issued, the card holder has the right to borrow any item from the Library's circulating collection.
Children applying for Library Cards must be accompanied by a parent or guardian with acceptable identification, or a parent or guardian with acceptable identification may apply for a card on behalf of a child by presenting the child's Social Security Card.
All library records are confidential. This means that your borrowing records will not be made available to any agency of state, federal or local government, or to any other individual unless a specific court order is issued, in accordance with the policies of Fort Berthold Reservation Library.
The only exception to the Library's confidentiality policy is the borrowing information on overdue items of a child under the age of 18. Because of the legal responsibility to pay a minor's overdue fines and/or fees, these overdue records will be released upon request to a parent/legal guardian.
Your borrower card should be presented in order to borrow library materials. Valid identification is required to check out materials if you do not have your card with you.
Please inform the Library if your card is lost, stolen, or if you change your address and/or phone number. You are responsible for library materials checked out on your borrower card.
Identification with current name is required to apply for a borrower card. All applications must be made in person. After application, the new card will be mailed to you or may be picked up on your next visit.
Materials from the circulating collection may be kept two (2) weeks and may be renewed if no one is waiting for the item. Renewals may be made in person, telephone, or by mail. Items must be brought into Library after second (2nd) renewal.
Patrons may only borrow material for themselves.
Each patron is responsible for material borrowed in their record.
No material may be taken out of Library that has not been checked-out
No more than four (4) items may be checked out to one patron in any one subject area at a time.
Materials will not be loaned to any patron who has overdue books and/or fines until materials have been returned or fines have been settled.
Video tapes are loaned for two (2) days, one per patron (i.e., out on Monday due Wednesday). $5.00 refundable deposit is required.
Signature on the library card application form indicates patron agrees to Library Policies and Rules.
